provider on your plan, please contact your insurance
company for verification.
Referrals
If
you participate in a HMO, MedicaidÍs Georgia Better
Health Care or any plan that requires a referral to
a specialist, it is our responsibility to see that
we have a referral authorization from your primary
care physician for each visit.
Insurance
Please
make sure you provide us with a correct and current
copy of your insurance card at your visits. We file
insurance strictly as a courtesy to our patients. We
ask that all co-pays, deductibles, and other percentages
not covered be paid at the time of service.
Payments
We
accept cash, checks or VISA and Master card.
Special Needs
It
may be necessary to set up a payment plan under certain
circumstances. Special needs are understood by this
office. Please contact our billing department as soon
as possible if this applies to you.
